MEMBERSHIP & SERVICE ELIGIBILITY
Who Can Become a Member
CAF membership is open to individuals and families who wish to participate in strengthening the community support system, as well as those who may request support from the community in the future.
Membership include:
• Community Support Participants — individuals or families who do not have medical insurance and may request assistance after meeting the eligibility requirements.
• Community Support Contributors — individuals or families who wish to participate and support uninsured members in the community.
Membership Structure
CAF operates through a recurring monthly contribution of $49, which sustains the Community Support Fund.
Each $49 monthly contribution is recorded internally as 1 Community Participation Credit (CPC), reflecting a member’s ongoing participation and commitment to strengthening the shared community support system.
Active membership requires timely, continuous, and uninterrupted monthly contributions.
Membership is voluntary and may be discontinued at any time.
Services Available Through the Program
Community Access Foundation supports access to the following basic services:
- Primary Care Visits
- Basic Vision Care (including one pair of glasses)
- Basic Dental Consultation
- Basic Legal Guidance
Members may receive supported services within a 12-month period as follows:
- Two primary care visits, or
- One primary care visit and one dental visit
- One primary care visit and one legal guidance session
FUTURE SUPPORT (FOR CONTINUED MEMBERS)
- Kidney stone support
- Pregnancy support
- Elderly care support
All services are subject to membership eligibility and are delivered through participating clinics and professionals.
Service Eligibility
- Eligibility for assistance begins after completing 6 consecutive months of active and uninterrupted membership.
- Eligibility is based strictly on continuous and timely monthly membership contributions. Lump-sum or prepaid contributions do not shorten or accelerate the 6-month eligibility requirement.
- If membership is discontinued, eligibility will restart upon reactivation, and a new 6 consecutive month period of active membership must be completed.
- Members who meet the eligibility requirement may submit a request through CAF’s structured review process.
- All requests are evaluated in accordance with organizational guidelines, the nature of the situation, accumulated CPC participation, and available resources.
- Support is extended in alignment with CAF’s charitable mission and subject to resource availability.
CPC Allocation
Members may allocate their accumulated CPC:
- For their own eligibility
- For an immediate family member
- For a friend
- Or to support another individual within the community
All CPC allocations are subject to internal review and resource availability.
